I've found quite a few good ones and then a few that let me down at the end with a miracle cure ending that precipitates the HEA. I recently read New Life by Bonnie Dee which has a hero with a permanent brain injury and a bad hip which I found very realistic and really loved. A classic I always go back to is Annie's Song by Catherine Anderson which is a historical read featuring a heroine who is deaf and mute. It's a real tearjerker!
